: Congratulations on your engagement and upcoming wedding! So that we are as prepared as possible, please fill out the following information sheet and return to us at least 72 hours prior to your showroom appointment.
Donation Request Form
: Liberty Party Rental takes great pride in supporting the communities we serve. Please find attached Liberty Party Rental’s donation/discount application. All applications must be submitted a minimum of 8 weeks prior to your event date to be properly reviewed and appear on our agenda. We understand your commitment and dedication to your organization and in order to give your request the in-depth review that it deserves, we must require this lead-time for adequate study. The Donation Committee will consider your application and you will receive a written decision within four weeks of being submitted.
Credit Card Authorization Form
: If you will be paying for your order by credit card, we ask that you fill out this form and return it to your account manager.
: Application for a credit account. Our payment terms are Net 30 and invoices can be mailed, faxed or emailed at the close of the rental contract. If you have a preference, please advise. Liberty Party Rental reserves the right to withdraw “on account” payment terms for non-payment within our Net 30 day’s terms.
Photo & Oral Statement Release Form
: Liberty Party Rental could not showcase our products if it was not for our clients sharing pictures of their magnificent events! If you would like to submit photos or a testimonial from your event to be selected to be showcased on our website or Facebook account, we ask that you fill out this form when submitting your photos and testimonials.
